Employees (faculty and staff) may contact Duke Access and Accommodation Services (DAAS) to request religious accommodations. DAAS will facilitate the process of identifying what, if any, reasonable religious accommodations may be available.
To begin the process, please submit the appropriate form below. One form is for DUHS employees who wish to request a religious exemption from DUHS’s vaccine requirements. The other is for all other religious accommodation requests.
After submitting your request, you will be contacted by a DAAS representative. You may be asked to provide additional information or clarification in order for the process to continue.
