The Disability Management System (DMS) provides leadership to the University and University Health System efforts to ensure an accessible, hospitable working and learning environment for people with disabilities while ensuring compliance with federal and state regulations. We serve as a centralized clearinghouse on disability-related information, procedures and services.

DEADLINES TO RECEIVE ACCOMMODATIONS FOR THE FALL 2015 TERM
I. STUDENTS ALREADY REGISTERED WITH SDAO:
SEPTEMBER 18, 2015: Requests for note takers
NOVEMBER 3, 2015: Semester Request Emails to Case Manager
II. STUDENTS NOT CURRENTLY REGISTERED AND RECEIVING ACCOMMODATIONS:
OCTOBER 20, 2015: Request Forms AND Documentation MUST BE SUBMITTED into SDAO
NOVEMBER 3, 2015: Meet and sign the Formal Accommodation Agreement
SPECIAL HOUSING REQUESTS
- Please note that the deadline for students to submit housing applications (assignments, reassignments, roommate pull-in) for the spring 2016 term is October 14, 2015. If your request for accommodations includes a special housing request, you MUST submit your request form AND documentation to SDAO by OCTOBER 6, 2015.
